Nutritionists at Health Strat Kenya

Application forms and prequalification documents are available from our offices at Saachi Plaza (Suite C8, Argwings Kodhek Rd) weekdays between 8.30 a.m-12.30 pm and 2.00p.m-4.30 pm., and upon payment of a non- refundable fee of Kenya shillings 1,000/= for each of the listed categories. Payment should be made in
the form of cash or cheque payable to Health Strat.

Applicants in each category will be required to submit copies of:

Company

• Certificate of Incorporation / Registration;
• PIN/VAT Certificate;
• Current Trade License / Business Permit;
• Certificates from affiliated regulatory or accrediting bodies/associations (where applicable);
• Current Tax Compliance Certificate;
• Evidence of Physical Location of business premise;
• Evidence of Electronic Tax Register (ETR);
• Three (3) letters of commendation from your corporate/major clients;
• Completed Confidential Business Questionnaire;
• Audited accounts for the last two years or 6 months Bank Statements;
• Company profile (inclusive of names of directors, management team and general structure of the company, number of permanent and temporary employees and any quality standard certification)

Individual

• Curriculum Vitae (clearly showing your qualification in relevant area)
• Relevant certificates (including professional membership)
• List (and provide evidence of services rendered) of 3 major references with their contacts within the last 2 years

Completed prequalification documents enclosed in plain sealed envelopes clearly marked as follows:

“Tender No……………………….. for………………………………………………………….” and addressed to:

The Procurement Committee, Health Strat, Saachi Plaza C8, Argwings Kodhek Rd. Nairobi

Should be delivered into the Tender box situated at Health Strat’s reception on or before Friday, 7 September, 2018 at 5.00pm.

Health Strat reserves the right to accept or reject any application and is not bound to give any reasons thereof.

Please note that Pre-qualification does not preordain a contract.
