Health Promotion Coordinator at Avenue Healthcare

Job Objective/Purpose

• Utilize a public health approach to create, provide, and coordinate wellness and health promotion, health outreach, prevention programs and services. To deliver successful schools health promotion and learner’s rescue program.

Key Responsibilities

• To establish successful and functional school health clubs in all schools involved in the program.
• To establish successful and functional Avenue Learners emergency rescue subscription service.
• To grow the number of schools involved in the school’s health promotion program.
• To grow the number of schools and lives subscribing to the Avenue Learners emergency rescue service.
• To leverage on the school’s health promotions program to create commercial opportunities and gains for Avenue Healthcare.
• Coordinate the planning and delivery of wellness programs including health awareness and educational campaigns, behavior change programs, health assessments and health screenings at all designated client locations.
• Understand and be able to evaluate the application of health communications, health behavioral theories and models within all clinical health promotion settings including coaching.
• Coordinate planning, delivery and monitoring of the chronic disease management program including its health awareness and educational campaigns, behavior change programs, health assessments and health screenings at all designated client locations.
• Develop partnerships with various stakeholders aimed at delivery of health promotion programs.
• Develop curricula for health promotion programs.
• Coordinate and offer training based on the health promotion programs to all stakeholders.
• Deliver culturally appropriate, research and evidenced-based health promotion, and employee wellness (fitness/nutrition/weight, life-cycle health, and disease management) programs.
• Plan events; organize screening days; and conduct outreach activities, which may include, but is not limited to conducting health screenings.
• Develop protocols and evaluation strategies for wellness program delivery, including (but not limited to) health screenings, flu clinics, physical activity classes, nutrition and fitness coaching, presentations/workshops, health promotion materials, and wellness events.
• Research and develop health promotion strategies (video content, mass messaging, etc.)
• Track outcomes, analyze data, and provide recommendations for program improvement.
• Reviews and evaluates available methodologies for Health Promotion programs.
• Maintains knowledge of current trends in the field of Health Promotion.

Personal Specifications

•  Bachelor’s degree in nursing, health promotion, public health, social work, nutrition, or related field of study preferred. 
•  Experience in health promotion, wellness programs or health education.
•  Relevant professional experience in human services field required
•  Minimum of 2 years’ experience preferred
•  Excellent interpersonal and communication skills
•  Attention to detail
•  Result oriented and reliable
