Overview

Pearl Hospital is seeking a motivated and experienced Community Health Programs Coordinator to lead the implementation of its community-based health initiatives. The role

involves coordinating field activities, managing community health staff, and ensuring effective delivery of care to clients outside the hospital setting.

Key Responsibilities
- Plan and implement community outreach activities including community NCD and home-based care programs in collaboration with key partners
- Coordinate and supervise field teams providing health services at community and household level
- Support patient follow-up, care continuity, and reporting
- Strengthen linkages between hospital services and community health structures
Qualifications:
- Degree or Diploma in Nursing, Community Health or related field
- Minimum 2 years’ experience in community health programming
- Strong organizational and leadership skills
- Experience managing field-based teams
