    Service Provider (Clinical Officer / Nurse)

    JOB SUMMARY

    She/he will facilitate the day-to-day tracking and clinical management of clients; coordinate and provide Key Population comprehensive HIV prevention package, including provision of HIV Care and Treatment services to clients. S/He will provide and lead biomedical interventions including STI screening and syndromic management, HIV testing services, contraceptive method mix, Cervix cancer screening, GBV prevention services, PEP and PreP services.

    KEY ROLES AND RESPONSIBILITIES

    1. Support the implementation of goals, objectives, policies, procedures and systems of the KP program.
    2. Participate in development of strategies for delivery of outreach activities including participating in outreach activities.
    3. Provide mentorship and build capacity of DICE staff and outreach teams in planning, implementation and supervision of KP prevention activities
    4. Provide clinical management of DICE clients, including ART services
    5. Work with the HTS department to strengthen both intra and inter facility linkage of the key population other services.
    6. Ensure 100% linkage of HIV positive KPs into care
    7. Ensure 95% viral suppression through timely viral load taking and intervention.
    8. Provide routine minimum package of service to both the negative and positive KPs within the Dice.
    9. Offer prevention services to clients i.e. SRH, PrEP, PEP, IPT, STI and TB screening and treatment MDT for unique and challenging cases, including clinical management of SGBV and CA Cervix screening
    10. Ensure 100% timely completion and submission of all clinical reports required at the Dice.
    11. Ensure up to date record keeping and management of client’s data.
    12. Monitor SSDs performance, documentation, and ensure monthly SSD reports are submitted.
    13. Prepare and submit regular program reports (activity, monthly, quarterly and annual reports) as per requirements.
    14. Document and disseminate program processes, outcomes, and lessons learnt and best practices
    15. Provide regular feedback/ update to staff and dice in charge on work related concerns
    16. Support and participate in Quality Improvement activities within the DICE.
    17. Work with site Quality Improvement team to come up with Standards of Care for the DICE.
    18. Support the Data team in conducting data audit within the DICE and ensure concordance of data.
    19. Oversee site data for completeness and accuracy.
    20. Participate in monthly work plan development and ensure prompt submission.

    DESIRED QUALIFICATIONS, SKILLS AND ABILITIES

    1. Diploma or Degree in Clinical medicine or Nursing
    2. Registered by Clinical or Nursing council of Kenya
    3. A minimum of two (2) years’ clinical experience in busy health facility, preferably HIV Comprehensive Care Center
    4. Trained in NASCOP certified HIV prevention, Care and Treatment programs including STI management
    5. Demonstrable experience supporting Key population clinical services in a DICE set up
    6. Strong interpersonal and coordination skills
    7. Ability to work under pressure as a team leader and player
    8. Good computer skills (familiarity with Microsoft Word, Excel and Power Point)

    Addiction Counsellor

    JOB SUMMARY

    As addiction counselling psychologist, S/he will provide psychosocial care and counselling services to patients who suffer from opioids/other drugs dependence. S/he will manage the psychosocial aspect of PWIDS DICE/MAT clients including recovery plan. The addiction counselor will also assist patients with the development of personal recovery programs that focus on addiction management, healthier behaviors, and harm reduction.

    ROLES AND RESPONSIBILITIES

    1. Meet clients and evaluate their health and substance problem.
    2. Identify issues arising and create goals and treatment plans.
    3. Update the clinic with clients’ recovery progress at multidisciplinary team meetings.
    4. Document patient interactions at the end of every session using the relevant forms
    5. Provide additional management programs based on identified concerns/progress.
    6. Evaluate the effectiveness of the treatment programs.
    7. Teach clients the coping mechanisms
    8. Meet with family members and provide guidance and support.
    9. Lead group therapy sessions.
    10. Guide clients re-establish career priorities.
    11. Refer clients to existing support groups, linkage forums in the community and follow them up.
    12. Undertake tracing of clients non-compliant to MAT or any addiction therapy.
    13. Lead community education and support programs for PWID.
    14. Oversee implementation of PWID promotional materials and information.
    15. Attend and contribute during site meeting within and outside the DICE or MAT clinic
    16. Ensure effective referral, linkage and follow up of HIV positive clients at the DICE and MAT center
    17. Participate in CQI and QA activities at the DICE and MAT center
    18. Adhere to the DICE/MAT SOPs and algorithm in delivering services
    19. Participate in support supervision
    20. Undertake routine data quality assessment
    21. Support in timely and accurate report writing on weekly and monthly basis.

    DESIRED QUALIFICATIONS, SKILLS AND ABILITIES

    1. Degree or Diploma in Counselling Psychology
    2. NASCOP certified training in addiction counselling
    3. At least 1-year experience conducting addiction counselling services to PWIDS
    4. Strong interpersonal and coordination skills
    5. Demonstrated ability to work under pressure as a team leader and player

    Adherence Counsellor

    JOB SUMMARY

    She/he will be key in supporting client’s retention on care and adherence to ART. S/He will also provide HTS services at the DICE including SNS, safe and ethical testing. S/He will plan and implement outreach activities.

    ROLES & RESPONSIBILITIES

    1. Attend and contribute during site meeting within and outside the DICE
    2. Offer quality integrated HTS at the site and in outreaches
    3. Ensure effective referral, linkage and follow up of HIV positive clients
    4. Effectively follow up index and social clients to reach their contacts – PNS & SNS
    5. Participate in QA activities-PT and internal quality improvement processes
    6. Adhere to the set SOPs and algorithm in delivering services
    7. Participate in support supervision
    8. Ensure proper waste management and cleanliness of the site
    9. Undertake routine data quality assessment per supervisor guidance
    10. Timely and accurate report writing, on weekly and monthly basis
    11. Do monthly summary for test kit used at the site and outreach.
    12. Manage RTKs (Rapid Test Kits) and ensure Infection Control and Preventions practices
    13. Conduct GBV screening and lead SNS activities
    14. Help client develop an understanding of their treatment and its challenges
    15. Prepare clients to initiate treatment
    16. Provide ongoing support for clients to adhere to treatment over the long term
    17. Facilitate clients develop good treatment-taking behavior
    18. Ensure appointment keeping and tracking of clients
    19. Conduct targeted home visits
    20. Maintain and update appointment diary and missed appointment registers
    21. Supervise ART client’s trackers

    DESIRED QUALIFICATIONS, SKILLS AND ABILITIES

    1. Diploma in Social or health sciences
    2. NASCOP certified training in HTS Counselling
    3. NASCOP certified training in Adherence counselling
    4. At least 1-year experience conducting HTS and adherence counselling services among PWIDS
    5. Strong interpersonal and coordination skills
    6. Demonstrated ability to work under pressure as a team leader and player
