    Nurse Navigator, Home Care Health Services

    Responsibilities:

    • Lead patient onboarding into the Home Health Program, including patient identification, home suitability assessments, care planning, patient and family education, and coordination of safe transitions from hospital to home.
    • Provide clinical leadership and oversight for home health nursing services, ensuring delivery of safe, evidence-based, patient-centered care.
    • Conduct supervisory home visits, support staff development, monitor clinical competencies, and coordinate ongoing training and professional education initiatives.
    • Coordinate multidisciplinary care, facilitate communication between patients, families, caregivers, and healthcare providers, and ensure an exceptional patient experience throughout the continuum of care.
    • Manage staffing and service coverage to ensure timely, uninterrupted care delivery while maintaining workforce compliance, licensure, and mandatory certifications.
    • Develop and maintain referral relationships with hospitals, physicians, and other stakeholders to support program growth and patient enrolment targets.
    • Lead quality improvement, patient safety, risk management, and clinical audit activities, ensuring compliance with organizational and regulatory standards.
    • Monitor clinical outcomes, workforce performance, and operational metrics, and prepare reports to support continuous service improvement and program effectiveness.

    Requirements:

    • Registered Nurse from a recognized school of nursing or University
    • Current licensure with the Nursing Council of Kenya
    • Certification in Basic Life Support (BLS)
    • Minimum of 5 years' clinical nursing experience in an acute care hospital setting.
    • Experience in Home Health Services will be an added advantage.
    • Demonstrated leadership and care coordination skills
    • Strong critical thinking and analytical abilities.
    • Ability to work effectively within multidisciplinary teams.
    • Strong organizational and time-management skills.
    • Ability to prioritize multiple responsibilities in a dynamic environment.
    • Self-driven with the ability to work independently.

    Manager Allied Health, Outreach Administration Office

    Position Summary

    The position holder will provide strategic and operational leadership for allied health services across the Outreach Health Network, including Pharmacy, Laboratory, Radiology, Dental, and Optical services. The role is responsible for driving operational excellence, service growth, quality improvement, regulatory compliance, and clinical governance to ensure the delivery of safe, efficient, and patient-centred care. Working collaboratively with clinical, operational, and support teams, the incumbent will enhance service performance, optimize resource utilization, and contribute to the achievement of the organization's strategic and financial objectives.

    Responsibilities:

    • Provide strategic leadership in the planning, implementation, and continuous improvement of allied health and outpatient clinical services.
    • Oversee the operational performance of clinics, ensuring efficient service delivery, optimal resource utilization, and achievement of key performance indicators.
    • Lead quality assurance and patient safety initiatives to promote excellence in clinical care and enhance patient satisfaction.
    • Ensure compliance with healthcare regulations, accreditation standards, organizational policies, and clinical best practices.
    • Collaborate with clinical, operational, and support teams to improve workflows, strengthen service delivery, and enhance patient outcomes.
    • Oversee Health Information Management Services (HIMS), including medical coding and central filing operations, ensuring accuracy, efficiency, and compliance.
    • Monitor inventory management in collaboration with relevant departments to ensure uninterrupted availability of clinical supplies and equipment.
    • Lead, coach, and develop multidisciplinary teams through effective recruitment, performance management, mentoring, and continuous professional development.
    • Utilize data and performance metrics to evaluate service delivery, identify improvement opportunities, and support informed decision-making.
    • Support departmental budgeting, cost optimization initiatives, and efficient utilization of resources.

    Requirements:

    • A Bachelor's degree in Pharmacy, Medical Laboratory Sciences, Radiology, or another allied health discipline, or a Bachelor's degree in a business-related field with relevant experience in allied health services.
    • A master’s degree in healthcare administration, Business Administration, or a related field.
    • A minimum of eight (8) years' experience in healthcare management, including at least three (3) years in a leadership role.
